Solid list, but somewhat out or order (i.e., a championing executive with success defined is going to help align goal, scope, PM selected, and so on).
At the risk of parsing words, getting everyone behind the project is not quite the same as roles and responsibilities.
And two to add, if you don’t mind:
#13 – There’s also the issue of a proper collaboration tool, which email is not. Not a PM tool, a collaboration tool.
#14 – Clarity & Understanding. That is, just because there’s communication (a means) does not necessarily translate (pun intended) to universal understanding and buy-in (the ends).
#15 – Be mindful of exceptions. An A+ results is not going to come from an C- commitment and effort.
Bonus: Never stop revisiting the rules even after the project starts.
