(Presuming you’re using Google Analytics, then search for ‘Google URL Builder‘.)

The more granular you are with your tagging the better. For example, the QR for magazine ad/location “A” should be different than “B” and so on.

If you’re not measuring (or at least attempting to do so), you’re not really marketing, right?
